Patient Registry

To use this population-based approach for care requires current and accurate information. It is critical that data be obtained and managed for each patient, and that a summary report of these data can be easily created to use in the weekly case review. Important data forms include a one-page Patient Tracking Chart that summarizes a patient’s chronic conditions, medications, and brief psychosocial information as well as the following DAWN intervention information:

  • PHQ-9 score at every visit,
  • Areas of focus for Problem Solving Treatment and Behavioral Activation,
  • Psychiatric medications and changes, and
  • Current stressors.

Another important report for clinical case review is the Caseload Supervision Form, which identifies for each patient for every visit the PHQ-9 score, treatment modalities (PST-PC or medications), and progress. This is a quick summary over time for all patients on a care manager’s caseload and can be developed with an Excel based form or Access database.
